So, you're ready to make some big changes and take control of your health and fitness? Awesome!
Before we get started though, I need to get a clear picture of who you are, where you've come from, what you've struggled with and most importantly what you'd like to achieve.
Everyone's level of commitment is different. Some people are just dipping their toe in the water. At the other end of the scale some people have hit that major snap point and they'll do whatever it takes to create the body they've always dreamed of.

What is your number 1 health and fitness goal? (Be specific. eg. "lose 2 clothing sizes in 6 months or less", "run a ½ marathon by the end of this year", "I have [insert medical condition here] and I want to cure/reverse the symptoms")
In my experience, successful transformations have only been achieved by people with powerful, emotional and personal reasons to change. It's one thing to make the decision to change, it's another to have a powerful, personal and emotional reason WHY you want to change. Why are your goals important to you?
On a scale of 1-10 how important are these goals to you, and why? If less than a 10, why?
What has prompted you to take action now? Has something happened recently that has made you "snap"?
What do you feel are the biggest challenges stopping you from achieving your goal?
What have you tried in the past to reach your goals? eg. Diets, exercise programs, gyms, Personal Trainers etc. Please give both positives and negatives of each experience
How long do you think it will take to reach your goal?
You are the most important investment you'll ever make in your life. Are you willing to invest in yourself to get the body, health and results you want?
I only work with people for a minimum of 6 months as I know through experience this is the minimum amount of time people need to stand a chance at permanent success. Are you ready to commit to yourself on this kind of level?
The support network you have around you is critical to your success. If you have a partner, do they know about your goals and if so do you have their full support with this? Are they aware that you are applying to work with us?
When was the last time you really did something for yourself?
Tell me about your eating habits? Eg. How many times per day, what are your regular meals, do you drink soft drinks etc...
How are your energy levels during the day?
How many times were you sick last 12 months?
